Our Response to COVID-19


Our office continues to adhere to our COVID-19 polices to keep our staff, patients, and their families safe.
Thank you for your understanding and cooperation as we do our part in keeping everyone healthy!

COVID-19 POLICIES:

  1. Please bring a face mask to your visit. Our medical office requires that everyone who enters has a face mask on.
  2. A COVID-19 questionnaire and temperature check will be taken prior to the start of your visit.
  3. Only scheduled patients are allowed into the office (exceptions: 1 parent of a minor patient, a caregiver, or a translator).
    Anyone exhibiting symptoms of COVID-19 or choose not to adhere to the safety policies will be asked to reschedule their appointment for a later date when safety protocols can be relaxed.

OUR SAFETY MEASURES:

  •       Acrylic barriers have been installed between patient and staff areas.
  •       Masks are required by all individuals who enter our office.
  •       Office Staff will have temperature checks prior to the start of their shifts.
  •       COVID-19 screening questions and temperature checks will be preformed for each patient.
  •       All patients will be asked to wash their hands upon entering.
  •       Our waiting room is now closed to allow for social distancing.
  •       As always, all equipment is thoroughly disinfected after each patient encounter.
  •       Our office is disinfected throughout the day and heavily disinfected at the end of the day.
  •       Our forms are available online to download and bring into the office.

PREPARING FOR YOUR EXAM:

  1. Please provide our office with your insurance information prior to your appointment day. Due to COVID-19 office constraints we will no longer be verifying benefits in office.
  2. Download and complete the following 2-page patient intake form and bring your form with you to your visit.
    At this time, your opportunity to elect whether you would like to have the OPTOS imaging scans completed with your exam will still be presented at the office.
  3. Confirm your appointment by replying to the Appointment Reminder text sent 1 to 2 days prior to your exam.
  4. Please bring your most recent glasses, glasses prescriptions and contact lens prescriptions with you.
  5. Bring a face mask to your visit. Our medical office requires that everyone who enters has a face mask on.
  6. Upon arrival to your exam, please wait outside the office in order to have your temperature taken and to answer COVID-19 related screening questions.
